Total Hip Replacement

Hip replacement is a procedure in which the native hip joint is replaced with a prosthetic joint. In India, this is normally indicated in conditions such as arthritis of hip due to various causes, avascular necrosis and also as a treatment of hip fracture.

The diagnosis is confirmed based on clinical findings and X rays. In some patients more sophisticated investigations such as CT/MRI scans may be required. Initially I recommend conservative treatment which includes medications, physiotherapy and activity modification. Surgery is indicated only if these measures fail.

 

The good news is that replacing the hip is one of the most successful surgeries in the history of medical science. 95% of patients are delighted with the outcome. I specialize in minimally invasive hip replacements and I also run an enhanced recovery programme for these patients. The advantage is that post-operative pain is less. Patients start to mobilise early and generally get discharged by 4 or 5 days after surgery. They have regular dressing every three or four days till sutures are removed around 14 days following surgery.

I recommend a daily walk with walking aids such as crutches or sticks. The patient can stop using this in the first few weeks once he/she is feeling confident to walk independently.

Choice of the prosthesis design is extremely important. I generally choose the design in discussion with the patient. I would choose a prosthesis that has established long term results as published in international medical journals. Actually there are only a handful of hip and knee replacements which have published 10 or preferably 20 year results. I would insist on using one of these tried and tested designs.
